Move-day forms: the interstate packet
The three interstate forms that live on a job, where to find them, their status badges, and how each exports on your company's letterhead.
Beyond the inventory itself, an interstate move needs a small packet of signed forms. Van Line Pro builds three of them on the job and exports each as its own PDF on your company's letterhead. This article is the map: which forms, where they live, and what their badges mean.
Important: These forms appear on a job only when the move type is Interstate. On a local or intrastate job, the Move-day documents section does not show. Each form also carries the header note "THIS FORM IS TO BE USED ON INTERSTATE SHIPMENTS ONLY".
The three forms

Note: The row on the job shows each form's short name and, beneath it, Form 1002, Form 1025, or Form 1023 as a reference.
Where to find them
- Open the interstate job.
- Scroll to the Move-day documents section.
- Tap any form to open its fill screen.
Result: The form opens with the job's order number, customer, addresses, and agent number already filled in. You only type what the app doesn't already hold.
Status badges
Each form row shows where it stands:
- Not started (gray) — nothing has been entered yet.
- Draft (amber) — the form has content but hasn't been signed.
- Signed (green) — the customer has signed. For the two-phase forms (1025 and 1023), the badge turns Signed once the customer signs at origin, which is the point the move-day packet is complete. Adding the destination signatures later does not push the badge back to Draft.
Each form exports its own PDF
Every form has its own Export PDF button. The PDF prints on your company's own branded letterhead, using the logo, company name, address, phone, license number, and USDOT number from your company settings. There is no vendor branding on the legal PDF. See Set up your company letterhead.
Tip: Fill and sign a form on the device, then tap Export PDF to share or save the finished document. The share sheet lets you email it, save it to Files, or send it anywhere iOS supports.
Good to know
- Signing a move-day form does not lock the job. Only Finalize & sign on the inventory locks it. You can keep editing the job and its forms after any move-day signature. See Signing move-day forms and Finalize & sign.
- Blank rows are dropped. Any entry row you leave completely empty is removed when the form is saved, so it never reaches the PDF.
- These are separate from the inventory. The inventory is its own document with its own signature. The move-day forms sit alongside it.
