Residential Property Damage (Form 1025)
Record damage to the residence itself at origin and destination, with photos and four signatures across the two walk-throughs.
Form 1025 records damage to the residence itself — walls, floors, banisters, doorframes — not the goods and not the packed contents. It is a two-phase document: you fill it at origin on move day and again at destination on delivery day, with a walk-through and signatures at each.
Important: This form is about the building, not the belongings. Furniture and box contents are recorded elsewhere. See Move-day forms overview.
Open the form
- Open the interstate job and scroll to Move-day documents.
- Tap Residential Property Damage. The screen opens with the title Property Damage.
Origin and destination
At the top, a segmented control switches between Origin and Destination. Everything below it — the entries, the checkbox, the driver number, and the signatures — belongs to the phase you have selected.
- Origin footer: "Recorded on move day at the origin residence."
- Destination footer: "Recorded at delivery, at the destination residence."
The Autofilled from this job section shows Order #, Customer, the Origin or Destination address (whichever phase is selected), and Agent #. Its footer reads "These print on the form automatically."
Record damage to the residence
Each entry is one damaged place and what happened to it.
- Tap Add damage.
- Fill the fields:
- Damaged property — placeholder "Damaged property — e.g. Hallway wall".
- Description of damage — placeholder "Description of damage — e.g. 3" gouge near baseboard".
- Optionally tap Add photo to attach a picture from your library. Once attached, the row shows a thumbnail and "Photo attached"; tap the trash icon to remove it.
The section header reads "Damage at origin" (or destination). Its footer: "Note damage to the residence — a gouged wall, a scuffed floor, a cracked banister. Photos are optional." Before any rows, it shows "No property damage noted yet."
Note: A row with no property, no damage, and no photo is dropped when the form saves. Swipe a row left to delete it yourself.
The "no damage" checkbox
If the customer confirms there is no residential damage for this phase, turn on No damage to my property. The footer notes: "Check the box only if the customer confirms there is no residential damage at origin" (or destination).
Beneath it, the Agent / Driver # field records the carrier rep's number for this phase.
The four signatures
Each phase has two signers, so the full form carries four signatures across origin and destination:
- Customer
- Carrier representative
For the selected phase, the Signatures section shows a row for each. Tap Customer signs or Carrier representative signs to open the signature sheet. Its footer reads: "The signature acknowledges the residential property condition recorded above." See Signing move-day forms.
The section header reads "Signatures at origin" (or destination); its footer: "Both the customer and the carrier representative sign after the walk-through of the origin residence" (or destination).
The release language on the PDF
The exported PDF reproduces the paper form's release verbatim, above the signatures:
"Signatures below acknowledge that the customer and carrier completed a final walk through of the residence. The customer's signature releases the carrier from any residential property damage not documented above."
The no-damage checkbox prints as "No damage to my property."
Status and export
The form's badge on the job turns Signed once the customer signs at origin — the move-day artifact is then complete. Adding the destination signatures at delivery does not push it back to Draft.
Tap Export PDF to render the full form (both phases) on your company's letterhead. Tap Done to save and close.
Common mistakes
- Filling the wrong phase. Check the Origin/Destination control before you type. Origin damage on the destination phase misfiles it.
- Checking "No damage" and still listing damage. Use one or the other per phase, matching what the customer confirmed.
